How many of your eggs got to blastocyst? And how many euploid/ normal from PGTA testing?

I've seen various drop off rates on the internet and am interested to know outcomes from real people.

My AFC is 20, my AMH is 12.5 and I am over 40. The clinic have suggested three rounds of short protocol maximim stimulation to hit my ovaries hard (embryo banking) + PGTA testing. I have only tried naturally for a few months and am considering skipping straight to this for time reasons. No male factor issues.

Is the below what I might hope for per round?

16 collected?
10 fertilise?
5 make it to blastocyst?
Maybe 1 euploid (I realise zero is a possibility)?

Am I getting the drop off rate at any stage wrong?

For those who are more seasoned than me:

  1. How many eggs were collected relative to your AFC?
  2. What percentage fertilised?
  3. What percentage made it to blastocyst?
  4. If you did PGTA testing, what percentage were euploid?
  5. How old were/ are you?

@Faith2024 how did you research the lab capability aspect? The appointment I had was with King's Fertility because their package seemed significantly cheaper than other places for what I need, but I don't want to compromise on outcome.

@Clocloxx did they offer any explanation for your unlucky response given your decent AFC and young age? Very best of luck next time round.

Hey sirensong they told me because of my low amh my egg quality will be bad. But I reached out to other clinics who may possibly think I could have hidden infections and want to do some testing and they said high stims was forcing my body to produce eggs which makes bad egg quality when my body just can not produce that many eggs 🥲
Thank you !

sirensong · 09/07/2024 11:34

@Clocloxx Despite your lower AMH your age still gives you an excellent chance if they can get the meds dose right. The consultant I saw last week said precisely this - that age trumps AMH by a significant margin.

Best of luck!

Yeah that's what all the fertility experts told me at the start, not to worry because at your age your eggs will be great quality! But after 2 rounds of failed ivf they changed there tune and said my eggs are just bad because I've low amh.
So I'm changing clinics and countries! Ireland has never known to experts in fertility 😑

Just had my genetic counselling session for PGTA testing. Attaching this chart of average % of normal embryos by age in case useful for anyone. Green is normal.

How many of your eggs got to blastocyst? And how many euploid/ normal from PGTA testing?

A potential plot twist is a very faint (maybe invisible on here) line just now... also a shadow on a boots early testing one. I'm likely 10 dpo.

If this is somehow real and not one of those famous evaps I'm actually not sure how I feel about it given the high miscarriage rate at my age. Whether it might just push back being able to do the banking and testing by a few months, meaning possibly worse stats.

@sirensong Looks like mine I had at the same time ☺️

I've just had a failed June transfer (fresh)

I'm 29, my DH is mid 30s. We've got a DC who is 2 and was conceived first month of trying. TTC for just over a year and just had our first IVF privately.
Results: 13 eggs retrieved, 8 mature, 1x day 5 embryo, 2x day 6 embryos. None euploid.

Those sound like awful results for my age?! AMH is 13 so not terrible. A million investigations have been done on myself and DH (hormone profile, hycosy, follicle counts, scans, semel analysis... to name a few), no issues found at all. Our diet is pretty strict, absolutely no drinking for both, no red meat for both, no added sugar for me... taking supplements - ubiquinol, pregnacare max before conception, omega 3 etc for me and Impryl, omega 3 ans others (can't remember all) for my husband.

Has anyone had a similar case? Do we need to adjust the protocol? Change clinic? What questions would you ask to the doctor?

sirensong · 29/07/2024 11:23

@strtng really* *sorry about the results from this round. Is there a particular reason why you were doing PGTA testing at your young age? Usually it's for those from late 30s onwards so wondering if there was a medical reason to suspect quality issues.

It may be that this round was just completely bad luck. Do you have others planned? If your local health board allows NHS funding despite private rounds maybe you should get a referral moving in the background when able.

@sirensong I looked into NHS but sadly not possible because we have a child already.
Tbh no issues, it's just that we really really want a child, to us personally the (financial) cost of testing was less than the (emotional) cost of going through failures and miscarriages.

strtng · 29/07/2024 12:22

@sirensong and yes, sorry forgot to add, we will 100% be doing another one, just not sure what changes to make to ensure better success next time - is it the clinic / something else etc.

Diet seems decent, neither of us eat red meat and haven't drank for about 1 year for me and 7mo for DH. I take ubiquinol, pregnancare pre conception, extra vit d. I don't have a stressful job at all (I'm a SAHM but have a lot of help)

Goldpanther · 29/07/2024 13:02

Amh: 9
AFC : 25
Eggs collected : 23
Fertilised with ICSI: 17
Day 5 blasts: 10

Fresh transfer was successful, currently 5 weeks, so hopefully things keep progressing well.

I found these threads really great to read whilst waiting/going through investigations etc so thought I'd add my stats.

Editing to add I'm 33, we had been trying to 7 years, got delayed for investigations due to COVID, and then had to try improve my DHs sperm count before IVF.

Can I join?

I was 37 at the time of egg collection, AMH of 30.2.

38 eggs collected, amazingly did not get OHSS after
Only 8 fertilized, I think a lot were immature
6 become blastocysts
After PGT-A my results were 2 euploid, 1 no result and 3 aneuploid

1st FET with euploid resulted in my 7 month baby boy.

Planning on transferring the other euploid next month, I'm absolutely shitting it!

browneyedgirl626 · 29/07/2024 21:36

Hi @strtng im sorry that your transfer was unsuccessful. Is it standard for your clinic to let embryos go to day 6? Just wondering as I think there is some research out there that says it's better to get them to day 5 rather than an extra day to 6. I can't remember where I read that though.

Does your clinic offer a follow up appointment to talk through how you responded to stims? Maybe see what they say. I think each egg collection can be luck of the draw I'm afraid for euploids/aneuploids.

I also am in the same camp with you re testing to reduce the chances of miscarriage. Nothing is really a guarantee but I did feel slightly more reassured being pregnant with a tested embryo especially after previous losses.

I think it's standard for our clinic to let them go to day 6, yes? From what I understand there's less chance that they're euploid but they might still be! And for some reason they said they're not always 'ready to test' on day 5.

My clinic isn't super responsive in terms of speaking to doctors but I've booked a follow up appointment for mid August.

Updating my own thread here. This was indeed a positive but it ended in a medically managed miscarriage at 9.5 weeks after many visits to the EPU. All fairly miserable but I'll take the positive that my body can get pregnant and that conception happened relatively quickly (4 cycles).

So I'm back to the original plan of 3 rounds of batching with PGTA to hopefully find a few needles in the haystack. I'll be able to provide my own stats in answer to my question at some point.

I'm currently doing my first round and am on day 6 of short protocol high stimulation (225 Gonal F + 150 Meriofert). At yesterday's scan (day 5 of stimulation) 20 follicles were seen but the consultant wasn't convinced the bottom two rows shown on this graph would end up large enough.

Did anyone's 6mm follicles on day 5 end up having a rapid growth spurt?

Anyone else want to add their stats?

I've done 3 rounds.

1st
Eggs collected : 7
Fertilised : 6
Day 5 / 6: 2
Euploid : 1

2nd
Eggs collected : 10
Fertilised : 8
Day 5 / 6 : 6
Euploid : 3

3rd
Eggs collected : 13
... waiting to hear more on this one.

Feels like for my age (late 20s) these are v low numbers and poor quality eggs.
AMH is 13.
