Can they really take responsibility for this? If someone is determined to take their own life then they will and we can do our best to prevent this but should we really take responsibility for their life?

If somebody wants to take their own life, do they go to hospital?

If you admit somebody to a hospital, you agree to provide some care.

So this is not the first.

I'm not saying that the hospital weren't negligent in this case, particularly as this has happened more than once.

However, suicidal people are often admitted against their will and even when they are in 'voluntarily' it is often because there is the threat of being detained under the mental health act if they don't. being admitted against their will could result in their being detained for longer, particularly if they are deemed to be lacking in insight (which can be ascertained by their refusal to voluntarily go into hospital....)

www.telegraph.co.uk/news/uknews/crime/9910273/Britains-failing-mental-health-system-must-change.html

According to this report, there were 12 killings due to possible failures in care provided by Oxleas NHS Trust during the last 10 years.

So no excuses.

This is very much like the huge number of death in a mid staffordshire hospital where several died, and the trust kept hiding the truth under silly investigations.

Criminal charges should be brought against those responsible.

Stephen Firn, chief executive of Oxleas NHS Foundation Trust, said: “I was deeply saddened by Mrs Mager’s death and would like to express my sincere condolences to her husband and family.

“We carried out a full and thorough investigation which concluded that it could not have been predicted or prevented.”

After 5 months, the trust accepts liability.

Angry Angry Angry Angry Angry Angry Angry

What about all the previous victims who lost their lives and truth covered up by "full and thorough investigation"

Who is going to look into previous suicides?

Who is responsible for the failure and coverups?

Accepting liability should lead to prosecution of those responsible

It is really sad to hear about the tragedy and loss of a precious life. It is equally infuriating to read so many news about the callous attitude of this NHS Trust. Reminds me of the North Staffordshire NHS Trust.

Several family members complain. The trust conducts their own "internal investigation" and cover up the failures. Poor family members have to get on with their lives and thus put up with poor care being provided.

The highly paid managers have to be investigated for these crimes.

Hope the local press continues to keep pressure on this trust to improve the safety standards. I agree that there should be a proper review of the senior management structure.

Oxleas NHS Mental Heath Service will never learn from their mistakes as they simply never want to listen to patients. The only time they sit up is when there is something in the newspapers.

A leading London Solicitor said this to me. 'The Mental health Service is very very powerful. They can do anything to you and get away with it.'

I know from first hand experience that the mental heath service has no real care or concern for patients. They do not like intelligent people. If anything they like to drug you up even more so as to show who has the power.

Mental health comes in all shapes and sizes and some people often just need some quiet time and rest. But Oxleas NHS think that everyone should be on drugs and believe me they love putting fear into peoples lives.

Neglect and cruelty will never go away and that is because they have all the power and the patients often only have a legal aid solicitor who is not really interested in their welfare.

OxleasNHS Mental Health believe that drugs are the answer to everything .....well maybe for them, but not for the patient.

It's a shocking catalogue of failure - and arrogance.
