Help with dereg letter?

Can anyone help with the wording for a letter to deregister DS from school? I know there is a good example on education otherwise but the website has been down this weekend.

Just to say the regulations you quote are slightly different depending on whether you are in England or Wales. (And obviously it's COMPLETELY different if you are in neither)
