Passport help please!

Can somebody help please?

I'm applying for my child's first passport, he had his surname changed by deed poll about 5 years ago. I'm applying for his first passport now and I'm so confused at the second section about a signed statement? what does it mean does anyone know? Thanks!!

Did the father give permission for the name change?

You need either the signed statement from both parents (if both have parental responsibility) or a court order. Look for a model statement on line to see what to write

Maybe write a statement saying both of you agreed to the name change and then both sign it.

Yes we both signed it and it's also our signatures on the deed poll too, we are still together and he is our son. Not really sure what to even write?
