Passport advise

I got married in 2004 and didn't get my name changed on my passport. Since then my passport has expired so obviously I need to get a new one.
Which box will I need to tick, the one to renew passport or one to amend details on the passport?
Will I be in trouble for not getting my name changed on it from when I got married?
Not sure exactly what it says on the paperwork now as it's not in front of me.

I don't think you'll be in trouble. I suspect you can renew and change details at the same time, anyway.

I would change the name in the passport. Even if it is just to allow me to buy tickets online (many companies ask for the cardholder to be one of the passengers).
Besides, it is a hassle trying to convince a minutiae obsessed officer that the woman in front of them, that is in the picture, that has the same nationality, and names, is still the same even when she omitted the second name.. forget about having a completely new surname.
