Parental Travel Consent Form

Do you have a good template for a Parental Travel Consent Form? My sister (different surname to me/DS) is due to fly from mainland Europe to the UK with DS (both UK passport holders). I want to make sure she has a suitable parental consent form. The only ones I have found so far are USA biaised. Alternatively do you have experience of doing this i.e. travelling with someone elses child??

My eldest went to the USA with his garndparents a couple of years ago .

I based my consent form on one that I had seen on the web , proably an American one .

I stated that we gave grandparents full parental consent and in the case of an emergency that had parental consent etc , included full passport numbers etc , dates of travel in and out of UK .

I also put on letter contact numbers for us including mobile .

Both my Dh and I signed the letter .

Thanks, seems like I'll have to cobble something together.....

I just normally get DH to write a letter, when I travel with our kids. (It should be ok, shouldn't it - but I don't have a UK passport, and I still have my own surname. And we travel to Canada, which is famously difficult about these things.)
