Travel health form do I put toddler and baby on?

My partner and I have the first 2 vaccines but not our booster. Its been over 270 days since second vaccine.

We have a 3 year old and a 1 year old. Do they have to be written on the form?

Also do I have to take a lateral flow test and upload a picture of it? I'm just wanting to know so I'm organised as it has to be filled in 48 hours before flying.

For the Spain Travel Health form you don't need to add your children under 12, no.

You and your partner will need to take either a PCR test 72 hours before arrival to Spain, or an antigen test 24 hours before arrival. These need to be paid-for tests from a company that will issue you a certificate that you can present at the health check, should there be any when you land.

Home LFTs are not valid, neither will be 'uploading a picture'.
