Passport - 16 year old travelling on child passport

Hi all,
I'm new on here and was hoping to get some tviews on this please
My lad turned 16 in Febrary and we are due to go on holiday in August so he will still be travelling on his child passport. I know this bit is ok.
I was chatting to the passport office about something else earlier today and this came up and they confirmed it was fine but they added "if the appearance has changed too much you might need a new passport for him"
I asked them to clarify this but but they were really vague and would only say if they are recognisable then it is fine.
He was 12 when the photo was taken and is just 4 months into being 16 now and he has a different hairstyle. Nothing really drastic to make him unrecognisable.
This is the first time this has been mentioned to me so I'm a little worried now. He surely doesnt need a new passport for this now?

I don't know the actual answer but my 4 year old travelled this year, his passport photo was taken at 6 weeks old so obviously he has changed a lot. No one said anything

He can use the child passport until it expires. The photo doesn’t matter.

Can anyone give me advice please? … My 16 year old daughter is classed as an adult by the airlines, BA & AA. She is travelling to the US with my cousin and her family, different surnames. My daughter is meeting cousin and her family at the departure gate as they are flying in from Ireland.
I am preparing a permission to travel document and naming my cousin as her guardian for the week. Does this need to be notarised?
Any other helpful tips welcome! 😊
