Sorry !!
Only done the first half of my passenger locator form to get into Spain as don't go until the 25th so cannot finish it yet.
Can anyone tell me what I'm going to be asked in this form? Do I have to somehow prove our vaccination status ? And do I need our day 2 tests codes/urn codes for this form

Also, do I need to register my day 2 tests before I go or just when I'm home and have done the sample ? Sorry lots of questions but my head is all over the place with all this admin

You will need the reference of your 2 day tests for your U.K. form when you return home.

For the Spanish form you need to give your seat numbers, the date you got your second jab and what make it was.
Then they give you a QR code for each person that you print off and show the Spanish authorities when you arrive.
Just did ours today.

The reference number for your Day 2 tests goes on the British plf that you fill in 48 hours before coming home.

Ahh thanks so much! I take it there is an option to miss out vaccine dates for kids as they're obviously unvaccinated ?

In regards to the codes for the day 2 tests, are they the ones that get sent in the confirmation email once ordered (the booking reference number) or are they the codes on the sticky label actually inside the individual boxes the tests arrive in?

@whatagloriousthingtobe, you need to check with the test provider.
Double check your emails from them to see if there's a code there first.
(I ended up calling mine as the reference number wasn't accepted for my return - it was a separate code.)
