First Choice have a SplashWorld range - ones we have been to have all been good - been to Magic Natura (favourite), one in Menorca (2nd fav), Estival Park (3rd fav) - close to Barcelona so might work with a 13 year old. Magic Natura also zoo onsite. Our kids where younger so Estival may be best of those three for a 13 year old, had a livelier feel.

I second Firstchoice Splashworld holidays.
Have a browse on the website (note: make sure you’re looking at Splashworld hotels, NOT Family Life hotels, as the latter is more geared at younger children)
We went to Hotel Marina Park in Menorca and DS loved it. The water park is onsite and is fairly new with some fantastic slides and a wave pool. It was never too crowded either, with no more than a few mins wait for the slides which meant DS was happy, and he made a few friends there. We also never had a problem getting sun beds. Food and drink at the hotel and waterpark is all inclusive.
If you don’t fancy Menorca there are lots of other destinations to choose from. Most waterparks are onsite within the hotels but a few are a bus ride away, so just check before booking.
We are going to a different one this year in Crete with DS (12) and DSS (14). They cant wait and neither can I. I know they’ll be so occupied with the slides that I’ll get a chance to relax and read my book in peace! Win win Grin

Salou.
Fly to Reus, transfers are quicker than Barcelona. There's a brilliant theme park and water park.

I'd go to your local travel agent, give them the dates you're available and a budget and get them to make some suggestions.

We've booked with Thomas cook past 4-5 years now and the staff are super helpful with deals and we've never paid full brochure price - we lived our holiday last year so much (me, dp & dd) we're now going with 2 sets of friends - have a look

São Miguel is probably the best for a family holiday. Ponta Delgada, the capital, is a good base for exploring both São Migueland some of the others islands, as island-hopping boats leave from here. This hotel is highly recommended www.booking.com/hotel/pt/quinta-do-carmo.en-gb.html and they have a detached holiday cottage which sleeps 6.
