I couldn't find mine and it had expired. I just filled in the form for a lost passport (I phoned first to check) and all was fine.

I got mine renewed in person and the guy said as long as it's reasonably recent (I think it was actually around 2000) then they could find the passport number on their computer.

They told me they only want the old passport because it saves them looking up the number.

I been there. I sent my application with my marriage certificate and that form. My boss when signing the photo did birth name/ married name. All came back all ok

I was in the same situation. Booked a last minute after a few glasses of wine holiday then realised my passport was lost and expired. The online options didn't seem to match my circumstances. In fear I booked an appointment at the local passport office and my appointment lasted a full 5 minutes.

They said I could have phoned asking what I needed to apply for (lost) rather than going in person. I missed out the part it was needed for the following week due to alcohol.

Oh and I found my lost passport the day after my appointment
