Child Travel Consent Form, Free, Downloadable?

Ds1 is going to France with his best friend and I have been asked to fill in a Travel Consent form. I went on line to try and find a draft that I could just easily fill in with all his passport details etc, but many of them cost £4.99 or £7.99
Please is there a FREE one that I can fill in and print off?

Thank you Ony, but they were one of the ones I was trying to avoid. It's says free, but when you painfully get to the end, having filled in all you your data, they charge - their one is £4.99. Others were £7.99

But thanks for trying. Don't worry, I've managed to use the basics and create my own now.
