Passport is on HOLD

Hello to All !
I have applied for New Passport and it is on Hold at Regional office-Surat due to 2 different name LC are submitted. Birth date is 19/12/1971 and Birth Place is West Bengal.

Pl guide me what to do ?

This is a UK based website so unfortunately I doubt that anyone on here will have knowledge of the passport procedures in India.
