Applying for baby passport, my passport in maiden name

Hi there

I may have asked this before and I am sure even if I haven't there is info out there if I search but it's far easier just for me to ask again and get the answers here so please forgive me.

I DO know that I don't HAVE to change my passport (phew - years left on it and waaay too expensive)

I DO know that I can fill in the form with my maiden name.

I DO know I have to book my ticket in my Maiden name to match the passport.

I DO know that it might be an idea to take my marriage certificate with me...

What I need to know is:

I have filled in the passport application for my son online and put my married name in, my passport has my maiden name on it my son's birth certificate has my married name on it. Do I need to write in the notes that my passport is actually in my maiden name? Or do I need to get another form and fill it in with my Maiden name?

I just did this a few months ago, same situation, my passport in my maiden name, DD birth certificate in my married name, you need to fill in the passport form with the name in your passport- maiden- and then explain in the notes, you will also have to send in your marriage certificate with the application.

The only thing I found confusing/annoying was they ask for your full name including Miss/mrs/ms.

I am a mrs but because I was putting in my maiden name I felt like I had to put Ms when I would rather have put Mrs or Miss.

Same as me. Filled out DC's forms with my maiden name (as that is what my passport is in), and wrote my passport number in the space provided. Don't think I had to give the marriage cert (although I can't swear on that).

I think I sent the DC's full birth cert, which had my married name but also my maiden name on it.

I can't remember any probs!

i had this and spoke to the passport office to clarify. I completed form in married name and signed as such as that is now my name. I then sent my maiden name passport and marriage cert with it.

you only need to take marriage cert on holiday if you and your son travel alone - i asked passport office yesterday about this and they said only one parent needs to match and its ok to be the father

You must apply in married name if that's your legal name. Sign the form in your married name, enclosing maiden name passport and marriage cert. in section 8 state you are still using maiden passport which is perfectly legal til it runs out.
