Sea life centre in Birmingham itself if weather bad
Black country living museum
Haven't been to dudley zoo for years, but when I went, it was the most depressing zoo ever.
Coventry (short train ride) has a museum of motor transport which is a big hit with my DS (5)

Cadbury World (not cheap and you will need to book tickets in advance if going in half-term, but my children aged 7 and 10 really liked it).

The Think Tank has a nice play area for young children.

if you have a car ...
hanbury hall
west midlands safari park
midlands art centre has a nice playground but busy
museum and art gallery
botanical gardens
seven valley railway
clent hills

The Nature Centre combined with the park and perhaps lunch in the Mac (the Mac may well have a children's film or show on too).

Walking in the Lickey Hills (the visitor centre has a good playground).
