I am currently enrolled on an access to health professions course and part of this involves doing an extensive research project. I have put together a questionnaire regarding this and it would be extremely helpful if people who have experienced premature births could take the time to answer this. The link to complete is below.

You have forgotten to ask the cause of premature birth, which will have a major impact on the answers about health concerns. I’ve filled it in though.

I started to fill it in but realised it was for only those babies who survived. Sadly, mine didn't but my intention was to help you out OP. Sorry I couldn't.

I am so sorry to hear this. The answers can be whatever you want them to be although it appears to be mainly focused on those that survived it would be helpful for research purposes to see those that sadly didn't

Thank you for this, I decided for the purpose of my research report to not ask this question as it may lead to a wider subject matter. However the reasons for premature birth may be explored at a later date

Do you want to know about multiple prem births? i.e where you have more than one child who was prem. The survey only allows you to fill in for one child.

Q3 & Q4 are effectively the same

I was going to complete this for you, but I had premature twins and the answers will be different for each. I suppose I could fill in a response for each of them if it will let me, would you like me to?

There are no options to answer differently for twins. No option to enter what week they were born. No option to answer properly if they did not require any additional neonatal care.

If you have twins please but two answers in each box. Thank you
