What does the G&T register actually mean?

My dd's school have told us that they want to put her on the G&T register as she is "streets ahead" of every other child in her year (last year she was assessed as a level 4 in English - she is 7).
My question is, what does that actually mean in practice? Do you get extra work? Different work? Are there any advantages when applying to/moving to secondary school? Are you externally assessed? Are you reassessed each year? Is there 1 register or different ones for different subjects?
Sorry to be so completely clueless and basic, but although of course I've heard of G&T I can't find out much about the realities (even from studying previous threads!)

Best ask them what it means Burroo, as it depends on your school and they all seem to differ hugely. Some do lots with the G&T kids, others virtually nothing according to some on here.
