Can you please explain fit note- period it covers & phased return?

I've had a few months away from work sick. When I last submitted the fit note request to my GP I added a comment about how if poss I'd like a phased return. This is the last fit note I'm expecting. It arrived this morning, goes up to 2nd November, ( all fine) but the box for phased return " if available and with your employers agreement" was not ticked.

I've been having regular contact with HR and various emails to and from have said that I would like this. No issues raised.

I called a friend who says that it wouldn't matter that that box was unticked because I can't even be at work until November 2nd. Yes, but surely this box is talking about when the fit note runs out.

Can anyone explain whether I do or don't need that box ticked? I've been told that Occupational Health will be involved so wouldn't they expect the last fit note to refer to a phased return?

If OH are involved then they will help support a phased return.
I’ve had an extended period of sickness before that required a phased return. My fit note was dated to my last official date off sick, so did not cover my phased return. That was discussed and agreed by my manager and OH.

I’m on a phased return right now, and as with the PP above, I came back after the last date on my note, and am now working mornings only as agreed with my line manager about a week before my note ran out. My GP told me that if I’m able to work out the phased return informally myself to go ahead, and if not come back and he will write something more prescriptive. He wanted me to be in control of the situation myself if possible.

Thanks @janoo - sounds like you don't think there's a problem here and like my friend says the phased return box doesn't have to be ticked on this one?

Thanks @WildlyInappropriate . Sounds like I don't need to worry and the box doesn't have to be ticked. Best wishes.

Your current fit note presumably says you're not fit to work - the phased return box would only be ticked on fit notes that state you are fit to work, but with adjustments.
