Sick note help

I had surgery last week and got a sick note from the hospital however I’ve noticed it doesn’t have a date on. It just says ‘6/52’ for 6 weeks off.

Can I add the date myself or do I need to go back to the hospital?

Can you check with your HR department whether or not that is ok for them?
