Thyroid medication change

I’ve been on 50 mcg Eltroxin since 2017 never been changed as yearly blood tests have been fine I’m told
For a few months I’ve felt very aware of my heart beating I wear a apple watch which shows my heart rate could be 90/100 sometimes more and I could just be sat watching television.

During sleep I have a continuous high heart rate last night was 88-110 but I’ve had up to 148 and 157 on two occasions according to the watch
I am very aware of my heart ticking in my chest, I feel dizzy a bit out of it at times but I have years of mild anaemia and I have low blood pressure so could be that?

I had a blood test last week and today the nurse rang me and told me I needed to take my 50mcg one day and 25 the next then she changed her mind and said I need to go on 25 daily..she then said she needed to talk to the doctor and came back and told me to only take my 50mcg Eltroxin every second day so I miss a day in between..I’ve got an appointment to see doctor on Wednesday..I’m a bit wary because nurse told me three different doses
has anyone taken their medication every second day like this?

I've had various combinations over the years eg 50 one day and 25 the next, but never to miss a day. Though I wonder if she said to take the 50 one day & nothing the next as you may not have 25mcg tablets and she wants the doctor to see you before they are prescribed?
Do you have 25mcg tablets? If you do, I'm not a doctor, but I'd take 25 a day until you see the dr on Wednesday - after all that still amounts to 50 over 2 days but more evenly spaced.

Thank you.. No I don’t have any 25 mcg tablets only my 50mcg..I thought it was strange to miss every second day so I’ll miss tomorrow Monday and Wednesday but at least I’m seeing the doctor on Wednesday and maybe she will prescribe 25mcg we’ll see .. I’m hoping my heart rate will settle soon
