One stop shop - is there such a thing

I very rarely go to the GP but have a few issues where I was hoping to get all looked at once . I have no problem with the GP but am aware it's hard to get an appointment and want more than the allotted 10 mins .
I'm happy to pay if there is such a place .
Issues - very painful finger which was dislocated 25 yrs ago and now cannot touch anything without horrific pain

Have never been on HRT but post menopause have a lot of soreness and general aches / pains , very dry , and discuss DEXA test
Dietary advice as I'm struggling to lose weight .
Possible hiatus hernia .

Anyone else need a health MOT .

Thankyou, I'm 59 and never been invited to one of these . There are a lot of important illnesses / diseases but It doesn't mention the things I'm interested in though so I think I need something individual to me .
