Eye Prescription help please!

Just wondering if anyone smarter than me could help me translate the details from my prescription into the fields on the website?

Right eye: sph +3.50, cyl -0.50, axis 70

Left eye: sph +0.75, cyl -0.25, axis 45

Addition +2.00

You haven't got your PD. They don't measure this as part of your standard eye test, it's only taken when you buy glasses. If you've previously bought glasses from your optician they might give you the old figure. Otherwise they'll measure it for a fee or you can find instructions online to measure it yourself.
