Persistent UTI

Hi - any ideas or experiences?
I'm 52, through menopause, HRT for a couple of years. mirena for the first time in April. Painful insertion.
Last week I developed cystitis on Monday, lower back pain, frequent urination, burning. Saw a GP via Zoom, late at night, antibiotics for cystitis for five days. Thursday (so over a week later) I still had constant lower back pain and frequent urination and slight stomach pain, back on antibiotics now.
But, the back pain is constant as is the constant need to wee/ feeling I need to wee. Back pain is really uncomfortable. I've been about ten times since waking up and that includes waiting because otherwise I'd literally be happy just sitting on the loo.

What could it be? I'm going to contact the GP on Monday again but I'm so miserable with it. No temperature but I feel grotty.

Possibly a kidney infection?

That's what I'm worried about. Or that the mirena has moved maybe? It's horrible.

Im so concerned now. I had a second lot of antibiotics for a kidney infection and it seemed to resolve but from this morning (they finished Monday) I'm going all the time, back pain, discomfort. Dip test at GP is clear but I'm so uncomfortable.
