What is John Hopkin's ACG on my NHS record?

Can anyone help me understand the John Hopkin's ACG system? I have a score of 10 (red) which I think means I have a high risk of needing hospital treatment.

Is that right?

I was recently diagnosed with mild osteoporosis but am healthy apart from that.

It's a healthcare analysis system. I think it means your a high cost nhs user?? I may be wrong but that's my take on it.
