Sick note ending… and then holiday

Can anyone please help. I’ve been off work sometime with a chronic illness.
sick note ends today. I had planned to go back ( on phased return) but I’m still not right so will push it back. But I have a two week uk holiday coming up. Question, can I have a sick note to end, and then take two week holiday, as holiday. Or does not being there to have a back to work interview muddy the water. If I take a longer sick note to cover hol too, it doesn’t feel right. ( they would have to give it me back) Hope that makes sense. Thanks.

I would have done your RTW interview when you went back after your holiday. I'd have then planned your phased return, determined by length of absence

Thank you so much for your reply. So I could take a note for two weeks. Hol two weeks. And then back to work/ phased return straight after. But I do need a phased return note doing?
And hope im right then!
If I’m not right though, it’s a break in absence, unless doc backdates another note two weeks earlier, and I’m not sure they backdate that much.
Or another option, note a month, if ok then go back straight back to work after hol. And say use hol allowance for the last two weeks.
but then I still need a phased return note to folllw straight on!
it’s giving me a headache!

I would speak to your manager and tell her you will report fit for work at the end of your 2 weeks sick note, and then take your 2 weeks holiday and be back.

She can then arrange your rtw and phased return

My phased return had to be accompanied by a fit note from GP to 'prove it was needed'.

If you've been off a long time I'd try and have a conversation with your manager/occupational health to get a proper plan in place for a phased return and how that might be managed (for instance, first week reduced hours covered by sick pay, but from then on annual leave as it is in our organisation).

I’ve had a two week note
and then taking holiday
and the new note after that will either be a phased return one, or a back dated sick note.
