Really worried about BT results

I've been unwell on and off for a while, breathlessness with no obvious cause, headaches, brain fog, joint pain, extreme fatigue and increasingly heavy periods.

I've had blood tests done, which were repeated as some levels were out of the normal range. I've been asked to make an urgent appointment with my doctor to discuss the results face to face.

When I look at my NHS app it says a lot of stuff I don't understand as not medically trained but basically says this about my bloods:

Platlet count: high
Red blood cell count: high
Mean blood cell volume: low
Mean cell haemogloblin level: low
Fibrinogen: high
Basophils were seen on blood film
Serum folate: low
Serum c reactive protein: high
Serum ferritin: low

It also had a note from the lab to my GP to say I need more tests to rule out myeloproliferative disorders and also to check for Jak2 mutation.

Does anyone have any idea at all what this means before I worry myself senseless?

It all needs interpreting with the actual numbers, the context of your medical history, any examination findings, the trends in the results etc. GP should be able to make more sense of it than anyone on here can.
