Issue with GP

Name changed as previous posts might identify me. I've asked my GP surgery if they can make a note on my file that I don't want a specific member of staff to be involved in my treatment going forward. For context, I have a chronic condition that can cause several other issues. The last couple of times that I've needed advice in these other issues or medication I've been referred to this particular member of staff through their online system. On both occasions it's transpired that incorrect information has been provided by the hcp. One piece of advice caused me to become unwell although not seriously.
I'm not having much luck with the practice with this request. I know I can escalate the matter but I wanted to know if my request is unreasonable or unworkable. The practice is medium sized and there are other hcp performing the same roles.

It’s perfectly reasonable. There’s no reason why they can’t put an alert on you record to ensure you aren’t booked with that clinician again.
Did the incorrect information you were given, and you becoming I’ll, get logged as a significant event?
Maybe ask for a chat with the practice manager to discuss the event and your concerns and about not seeing this person again.
