Need help with diet for type 2 and cholesterol

Please could I have some help with what I should eat for each meal.
I just got diagnosed type 2 diabetes, need to lower my cholesterol, have a heart condition, am on 4 meds now for heart and diabetes.

I don't eat meat except chicken and salmon. Don't like mushrooms, egg or eggplant. Fairly lactose intolerant.

The nurse said low carb but also to swap bread for corn thins etc but then looking at the packet, the carbs for corn thins are much higher than bread. And she said wholemeal bread not grainy but again the carbs are higher on the wholemeal.

The sugar is less though.

I'm very confused about what I should be eating and looking for.
Is it sugar, carbohydrates or both.
I've tried googling for cookbooks for diabetes and can't find any thing.

Old diet was mainly toast or fruit for breakfast, baguette or cheese and onion pasty or doritos and hommos for lunch, pasta or rice for dinner with vegetables in. Sometimes chicken curry or salmon. Roast chicken on Sundays with everything.

I don't drink alcohol but do drink a lot of coffee and diet cola.

I was told to eat brown bread as it doesn’t cause a sugar spike. Diabetic nurse told me white bread gave you a sugar spike. Brown bread the sugar is released slowly. Lots of veg. I have just recently been diagnosed also.
