I have (what It says is) full access to my medical records online through my gp practice.
I am due to have a medical for coram (fostering/adoption medical).
part of the form asks about mental health and I don’t know how to answer.
I was put on sertraline in January 2021 for covid anxiety - it was a phone call about blood test results and I just mentioned it at the end and was prescribed 50mg of sertraline. Still on it now although haven’t taken it since June and would like to get it removed from my repeat.
On my notes for that day it it says nothing besides “ blood results” and then just says sertraline 50mg.
There has been no diagnosis of anything mental health related and no follow up regarding the Medication since I was put on it.
not sure what to write on the medical forms.
