How do I request my medical records

Hi
Does anyone know how I can get a copy of my medical record's please?

I've got 3 hospital consultants who can't agree on a diagnosis plus I've applied for PiP today as I've had to employ home help due to whatever I have leaving me disabled and blind in one eye . I don't have a definitive diagnosis, one consultant thinks I have TB of the spine , after I tested positive for quantiferon, another thinks it's auto immune (MS or similar) and one is keeping quiet .

You need to put in a subject access request through the Disclosures team for the NHS trust

No, because the GP and the hospital trust are totally separate organisations. PALS at the hospital should be able to guide you.
