Prescribing error?

I requested to reduce my dose of HRT from 100mg to 75mg more than a month ago. I had some 75s but there was a delay prescribing due to a suspected blood clot, so I’d completely run out by the time they were prescribed.
GP left a voicemail to say he’d prefer me to drop to 50mg but happy to try 75mg first. I picked them up and they were 100s.
NHS App has recorded 75s prescribed, but labels are clearly 100mg.
Does this look like a GP or pharmacist error?
thanks

Sometimes it takes a few days for new prescriptions to reach pharmacies. Have you spoken to either about it?

If the app says 75 and the box says 100, it sounds like the discrepancy has happened at the pharmacists, but like I said, they might not have been aware of the change.

are they patches?

They are patches. Weren’t on repeat.
I’ve emailed surgery (only noticed yesterday evening) but wondered if should also contact pharmacy.
I totally get it’s not life threatening but still frustrating when I’ve waited so long.

Could it be an old prescription? Ie. One the gp surgery had sent through before the change of strength.

Does the strength on the box match the label? If different, then that would be a dispensing error and to contact the pharmacy.
