Sore bottom - any ideas?

It’s really hard to explain - after a week of pile cream not helping I have seen a gp and have been prescribed some stronger pile ointment - she couldn’t see/feel anything but said that they could still be there regardless so to try the cream. I have only used the cream once as it made the skin so sore I can’t use it. She said I can get a referral if the cream doesn’t work which I will obviously do. It’s so hard to describe but it’s like a burning tingling feeling. Could it be stress?
I have ibs and have had flare ups recently so lots of loose stools. I’ve recently had a FIT test too and an ultrasound to check gallbladder, liver, ovaries etc. which was all fine so that’s why they suggested ibs. I just want it to go away!

I use germaloids as it has a local anaesthetic. If the itching is bad, I'll take an antihistamine.

I do have piles, and the bastards need to fuck off. I'm not keen on the surgical option right now.

Thanks. The thing is the gp couldn’t see or feel any so I’m doubting I have them? unless they are internal but I’ve read they are painless?
