Could I have had a 3rd or 4th degree tear (over a decade ago) and not been told about it?!

I've just had some test results back that include details of scarring that suggest i must have had a 3rd (or even 4th, not sure) degree tear during labour. I've never been told this and my youngest is 11!

I had two difficult births, forceps involved both times - 1st labour ended up with an emergency cesarean after failed forceps (and a small tear that I was told about at the time) 2nd birth was a VBAC and I was told I'd needed an episiotomy which I remember being very uncomfortable (had an infection) afterwards.

I presume it must have been the 2nd labour that resulted in the repair work that's now being reported - I've requested my hospital notes to try to find out but I'm really quite shocked that this could have happened without me being told. I've had issues ever since which I'm finally making progress with, but whenever I've given my history I've obviously not been giving the right information. I can't think of any other explanation but I also can't believe I could sustain this injury, which was repaired so must have been known about, but never told?? Has anyone heard of this happening?
