How to pace myself

My health is pretty poor. I have a number of long standing issues (diabetes, hypertension, liver disease, blood clotting disorder etc) that I'm managing as best I can.

The biggest issue I have that impacts me day-to-day at the moment is massive fatigue, muscle-aching and lethargy. I find that no matter what kind of day I have, by 3pm I am so tired I can hardly function. Even though I have a desk-based job I just struggle to keep going.

What can I do to help myself? I don't drink alcohol or caffeine, try to eat well. I don't exercise as I don't have the energy & also have health issues (back, hip, shoulders) that limit what I can do - can't even do yoga/Pilates at the moment.

I've had various tests for vit D, thyroid levels etc but everything comes back clear. Im late 40s and on HRT which has helped other symptoms but not these.

Any advice please? I have young children & feel like im failing them.
