Ongoing bladder issues

Swear I'm not a troll, just looking some advice on where to go next. So I've always had bladder problems from childhood, frequent UTI's, accidents and other times hesitancy.
I had an ultrasound back in March that showed I'm not emptying my bladder fully and I've been having pelvic floor physio for around 4 months. The physio has said that my muscles are very weak, but also tight in that they aren't relaxing properly.
Has anyone else been in a similar situation, I'm in my mid 20's and my bladder is so unpredictable. I'm having to wear pads all the time and I have accidents most days but then interspersed with retention in the same day. Thanks
