What does a hearing aid prescription look like!

Hi

I need a copy of my hearing aid "prescription" to apply for a railcard - silly question but does it look like a prescription? Does it say "prescription" on it? Or could it be something else in the paperwork I received? Private audiologist rather than NHS.

I would guess it means your audiogram?

Gosh, I have hearing aids too and have no idea either. Do they just need proof you have some? I can only think of my invoice for that. I’d probably email my audiologist and ask if they could supply something.
