Could be a first herpetic infection - ie coldsore
Apparently the first time you get blisters on the tongue, a burnt feeling in the mouth and you go off your food.
This si what my dentist said when we all got this - I am dubious though as dh and I have had coldsores before.

i'd take him to the dr to get him checked out

You don't always get blisters with the first cold sore infection, although it is a possibility.
Combined with his temperature, I would say hand foot and mouth. Sometimes the spots/blisters can just appear in the mouth; sometimes just on the hands or feet.

yes, hand foot and mouth.
the blisters come first iirc
