BIRADS-4 results, 9 years after bilateral breast cancer

Hi! I'd apreciate any help i can get because i'm very anxious right now. My dad had synchronous bilateral breast cancer 9 yeas ago. It was not agressive and it was in situ (was still encapsulated). He had a bilateral mastectomy and didn't need chemotherapy or radiation therapy and was fine after this procedure. Has lived a healthy and very nice life since.

Lately he has been having a "strange" sensation on his armpits and went to a consultation with his mastologist who ordered a mammary ultrasonography. We got the results today and it's BIRADS-4. I understand his doctor will probably ask for a biopsy now. Am i right? The cancer is probably back, isn't it? I'm just so nervous... Dad is not even 70 ans i haven't seen him since 2019's Christmas due to the pandemic. I miss him a lot and don't know what to do.

Oh, and i don't live in the UK or Europe, so diferent health systems.
