Severe osteoarthritis in ankle: absolute AGONY at Night!

I have had osteoarthritis for many years, as I grow older it is getting worse.
I have it in wrists, shoulder, lower back, hips, both knees, ankles and toes. GP said X-rays show severe osteoarthritis.

2 years ago I fell down the stairs into a stair gate and tore the meniscus in my left knee. It’s been agonising but after physiotherapy and a steroid injection it has eased. Obviously the knee injury had an effect on my gait and my right ankle is now painful all day and absolutely unbearable at night. If I sit still for any length of time, it will not bend when I stand and I cannot put any weight on it.

I have to walk as I have to work, I manage a small charity which will not be able to open if I don’t go in. I love my job so much but I dread walking.

I take dihydrocodeine, naproxen and paracetamol. I rub a freezing gel into it on dr’s advice. I wear an ankle support every day and a tube support at night. I raise it on a pillow.

Last night I had a bath and was in bed, cwtched up with a hot water bottle, having taken some anthologies, ready for sleep by 11pm. At 4 am I was still awake, in agony and I must admit, in tears. I was up at 7am for work.

What can I do tonight? My ankle has already started aching, putting it on the footstool makes no difference. It feels like a hot, huge lump of pain at the bottom of my leg. I have tried tramadol and amitriptyline but don’t like the side effects. If anyone says they will work I will try them again, in fact, I will try anything!

Sorry for that whinge fest, thank you if you got to the end! Please contribute your ideas

You need to see an orthopaedic foot and ankle surgeon.

Whether or not they can do anything surgical in the near future will depend where in the country you are, but most places are still able to offer treatments such as splints, physio and injections, plus, best to get the ball rolling now, especially since it seems you've tried almost everything you can do yourself.

If you tell me which is your nearest big hospital I may be able to recommend some surgeons for you.

I have been referred to Royal Gwent’s orthopaedic department but obviously there are delays at the moment.
I am also close to Cardiff and have been sent to Morriston in the past.

I was treated for cellulitis in this ankle in the past but was then told that it wasn’t cellulitis, so I don’t know what was going then.

I am getting ready to start the drug regime, wish me luck.

Agree you need a referral.

Also ask for a pain clinic referral and see if your GP will trial you on gabapentin - it can be useful in chronic pain

I am very lucky in that the medical centre I am registered with is truly excellent.
They are happy to take on board suggestions from patients. I might not say MN said... though I wouldn’t be surprised if some of the staff are on here. 👋🏻👋🏻

Different arthritis (RA) here but I find heat much better than cold for my pain. I use an electric heat pad plus I'm on Celebrex (anti inflammatory) as well as co codamol and hydroxychloroquine.
A Deep heat type rub might help?
I've just started using an acupressure mat Shatki I believe it's called which is meant to help with pain all over and also improve sleep, I've heard great things!

I had this freeze gel so I have been using it up, also it’s not quite as stinky as Deep Heat. I have a pack that you heat or freeze so I will try that too.

Sending a cwtch to you and to everyone living with chronic pain. I have had trouble for 30 years but in the last 2 years it has become unbearable. I hate when I hear myself snapping and being narky.
I was politically active but I struggle to find the patience I used to have so I think it’s time for a change of focus.
