I know one in and 1 out can be painful...is it wise to have 1 removed and a new one inserted at the same time?

It wasn't painful for me 1st time does that mean it won;t be this time.

Have got day of work just in case its a bit sore.. told boss needed time of for a minor op... he asked why then v quickly realised he shouldn't!

I didn't find it any worse than having the first one put in - day off work is a good idea though, I had crampy pains for the rest of the day.
