My 20yo dd1 has had sinus problems for ages, and also painful teeth. Her dr put her on a nasal spray for rhinitis and wow, what a difference to her sinus problem!! She no longer sniffs all the time and her speech no longer sounds as though she has a Grade A cold. As your ds's migraine seems connected to sinus, maybe a nasal spray may help?

yes, I would second suedonim's suggestion.

See if the GP can come up with something or get him referred to an ENT.
