When an internal investigation is conducted - what happens?

My treatment is now being investigated by a hospital. I will receive a response to their findings by 14/11.

I'm just wondering what do these investigations usually look like?
Is it Dr. A didn't do X, Nurse B didn't do Y?
What can you do if they find that there was negligence?
