getting copies of medical notes and medical photos

Has anyone done this? How much does it cost , how long does it take and who do I request them from? Very long sorry why but I need to transfer my son's care from one hospital to another and want all the info possible. Also priviate copies of the photos taken of his feet to show the original deformity and key points in his treatment.

I requested my hospital notes after birth of DS4. I phoned hospital and was put through to records dept. They sent a form to fill in with all details, think I had to send proof of ID and it cost £10

I looked on my local hospitals' website and found out how to request my records (think I had to make a written request, was then sent a form which I had to fill out and return - along with a £25 cheque).

The form asked what specific information I required, so I made some specific requests, but also finished it off by putting 'all notes and information regarding the pregnancy, and labour of xxx, and the birth of xx'.

I just wanted to make sure that everything was copied and sent to me - call me suspicious!
