Any hypothyroid experts able to comment on my latest blood test results??

I've been feeling crappy for ages now, a year or more, but bloods always come back in the normal range so GP has kind of given up and declared that I have chronic fatigue (fatigue is my major symptom to be fair.) However I have noticed that my TSH has been steadily increasing over the last couple of years. And todays blood test results show it is now out of range. I am convinced hypothyroidism is my problem, it is in the family for one thing, and symptoms fit for another. Todays' results:

Thyroxine (T4)
69.00 nmol/L (59-194)
Thyroid Stimulating Hormone (TSH)
4.58 mIU/L (0.27-4.2)
Free Thyroxine (T4)
13.90 pmol/l (12-22)
Free Triiodothyronine (T3)
4.30 pmol/L (3.1-6.8)
Anti-Thyroid Peroxidase Antibodies (TPEX)
13.60 IU/ml ( 0-34)
Anti-Thyroglobulin Antibodies (TGAB)
9.90 IU/mL (0-115)
Would that be enough for the GP to investigate further?? Any thoughts would be really appreciated.
