Migraines - topiramate - brain fog and exhaustion

After over 2 decades of migraines I'm waiting to see a neurologist at the headache clinic, but there's still another 20 weeks to wait and I don't even have a date yet.

I'm on 200mg topiramate a day for migraines but still get several a week so it's not really doing a lot at the moment. The dose had been increased over a year or so and in that time the brain fog has gotten worse and worse, I'm constantly exhausted and sleep through alarms, am late for work, my work is not going well as the standard isn't up to my usual level and is often submitted late. Fortunately I work flexi time so I get away with it but it means working weekends to make up lost hours so it's a vicious circle.

I had a full run of bloods last week which have all been signed off as "satisfactory" by the GP. This doesn't solve anything, and I've had to ask for a follow up as it was going to be left.

I'd be really grateful if anyone has any information or an opinion that could help me when I speak to the GP next week on what I should be asking to happen next, as I strongly believe there is more to this than meets the eye.

Do I refuse to take this stuff any more? I've been through just about every preventative there is and all were either ineffective (beta blockers, might as well have been sugar pills) or had scary side effects (pizotifen).
