How to get out of this sink hole

Lots has been going on the past few years. Lots of bereavements , involvement with Social services , court, you name it I've been there for some reason. Add to that an abusive childhood and you have a real bingo card of mental health issues.
I've made an emergency GP appt today, what should I be saying to them?

Oh bless you, you poor thing. How did the appt go? Have you had anti-depressants before as they may really help you?
I’m not sure if you have to give a reason for being signed off, I suspect your doc could just say you’re signed off with stress?

Thanks yes I had an appt and got given sertraline and a referral for bereavement counselling. Been signed off for a week , was offfered 3 but daren't stay off work for that long.
