3 year old's penis

I have noticed that my son's penis bulges at the end when he does a wee, it looks like his foreskin is filling with urine before the urine can get out.

Is this unusual, do I need to take him to the docs?

my ds had balanitis (swelling and infeciton due to stuff under foreskin

and hasn't had a peep of it since homeopath advised us to start (v v v gently, have i emphasised that enough yet?) to do that in the bath every night.
