Lifestyle advice needed from those with chronic illness

I have cyclothymia (or bipolar lite) and fibromyalgia. The doctors now think I have MS and possibly a heart thingy. Cue lots of strange seizures, constant eye-watering pain, painkillers which make me forget who I am, embarrasing forgetfulness and debilitating fatigue. I know how to have fun!

I am honestly struggling to run a household with three children (9yo but with severe ADHD and aspergers, 6yo and 1yo). I would love some advice on how to hold it together. Is it even possible? I thought simple meals, a bit of planning etc but this is hard when I struggle to make a cup of tea some days. I am not a domestic goddess so I am hoping that someone much wiser than me will be along soon with an action plan. It would be very much appreciated.
