gp writing a letter to say they are concerned about you

Got one the other day. Is this common? I feel really nervous going in!

Sometimes. I had a call from the out of hours after I had a blood test to see if I was OK (iron was a 'tad' low).

Do you have a long term medical condition that's not very well controlled currently? Have you missed an appointment, or do you have an ongoing prescription but sometimes forget to take it daily, so don't pick up your prescriptions as regularly as they might expect?

Is this a letter for your clinic specialist? If it is it could be a standard way of the GP asking the consultant to revue you in clinic again, and not wait for a perhaps distant routine appointment.
As Monty says it might be triggered by using more of a particular prescription than a person with a stable well controlled condition would need.

Is your telephone number with the surgery up to date? People change their mobile numbers like they change their underwear these days and don't advise.

If eg you have put in a prescription request and the Doctor needs to see you before issuing it or maybe because the item is discontinued or you need a blood test repeating and they can't get hold of you by phone they sometimes send a letter. There could be many reasons why you have received the letter. The only way to find out is to ring the Doctor's surgery.

There have been some changes in recommendations for asthma monitoring, so that using more than a certain number of reliever inhalers (I can't remember how many) a year should trigger a revue. Could it be something like that perhaps?
