Diagnosing rectovaginal fistula? Pregnant

After a run @24wks pregnant (#4) i discovered iv a rectocele. Pessary was inserted and it hasn't been particularly symptomatic.
I'm now 37wks and this morning an hour after a BM I discover feces upon wiping ...after much 'cleaning' I discovered it was from my vagina and not back passage. Was totally shocked, in my innocent state I didn't know it was even possible! Anyhow went to maternity unit where they did trace&scan (all great with baby) and a physical exam with speculum. Registrar found no tear in the vaginal wall and said everything was 'very clean'. She suggested it was old blood from a show I had last week..but I know what I saw, and I know where it came from ( for the love of God...I sniffed it, all dignity is dead at this stage!!)
My question for those few with exo of this is, is a physical exam the best way of discovering a fistula ?

You have been examined and there is no hole there.
There must have been a 'wiping error' when you had a BM.
It therefore should not happen again.
